Abstract

Treatment adherence is imperative for the efficacious psychopharmacological management of mental illness. Many factors influence adherence, but a patient referred to our regional affective disorder service raised an objection we had not previously encountered: vegetarianism. We sought information about psychotropics that might be acceptable to this patient. The suitability or otherwise of a medication for vegetarians is determined by its excipients and so varies by preparation as well as brand. Hence our survey was restricted to proprietary psychotropic medications. In April 2007, we sent a standard letter to the Medical Information department of the manufacturers of antipsychotic, antidepressant and antimanic drugs listed in the British National Formulary. No capsules were suitable for vegetarians, although the manufacturers of Duloxetine stated in vitro testing had demonstrated that the duloxetine enteric-coated pellets contained within the capsules are stable at room temperature for up to 2 h when mixed with either the apple sauce or juice, which quantitatively allows delivery of the full capsule dose. This case demonstrated the importance of inquiry into dietary choices that may impact upon a patient's decision to take medication. Switching to a different brand of drug, or administering the same medication in a different form may solve the problem. In psychiatry, it is also sometimes necessary to enforce medication, raising additional dilemmas.
